• Signed Consent for Hospital/Medical Center Photography & Publication
• Signed Team Member Acknowledgement of Confidentiality, Conflict of Interest and Corporate Compliance/Code of Ethical Conduct
• Signed FMLA Rights Acknowledgement
• Signed Acceptable Use of Policy Acknowledgement
