How to set-up an appointment for Rehabilitation Services
When making an appointment for Physical Therapy, Occupational Therapy
or Speech Pathology, here are some things to know that will assist us in
getting you scheduled in a timely manner:
Prior to an appointment
- Make sure you have a prescription (from a Florida Licensed Physician)
that includes a diagnosis and type of treatment that you will receive.
- Have you insurance information ready. If you have private or
commercial insurance, please be aware of your coverage and any
authorizations that may need to be obtained by your physician or
yourself.
- Photo Identification
- Have your own personal schedule available.
