Staffed by a Team of Clinical Experts

The Pavilion Sub-Acute Rehabilitation Center is staffed by a team of clinical experts including a medical director, physician assistant, nurses, therapists, dietitians and social worker. Together, with input from the patient and family, this multidisciplinary team determines the most effective and efficient treatment plan to maximize the patient’s independence. This plan is monitored and adjusted throughout their stay, and is always under the direction of a physician.

The Pavilion Sub-Acute Rehabilitation team includes:

• Medical Director and Physician Assistant: Our medical director and physician assistant work closely with the team to oversee the care plans of each patient.
• Nursing Care: The Pavilion proudly holds certification in the highly regarded, innovative and outcomes based NICHE (Nurses Improving the Care of Health System Elders) Program. They also hold a variety of individual nursing certifications and are held to the highest standards of competency.
• Therapy Team: A team of physical, occupational, and speech-language therapists work together to develop a comprehensive, individualized treatment plan to meet the needs and goals of each patient.
• Social Services: A social worker is available to coordinate care and to assist with discharge planning.
• Dietary Needs Team: A dietetic technician and registered dietician oversees each patient’s nutritional needs.
