What is a health navigator?

A navigator is a registered nurse who works to ensure you receive the right care at the right time. Our navigator can assist you and your family in manageing your healthcare resources. Our goal is to improve the quality of care you receive by helping you "navigate" through a complex health system.

Why do I need a navigator?

Sometimes healthcare is confusing. You may have questions and would liek to talk with a healthcare professional you can trust, who will listen. Sometimes you need help findina a medical specialist or need to undersatnd what screenigns or tests will help you stay healthy. Our navigator is just a phone call away.

What services does a navigator provide?

Our navigator offers one-on-one, personalized support. Here are just a few of the things our navigator can do for you:

  • Research questions and coordiante care related to medical conditions
  • Locate area resources and act as an advocate
  • Provide information on preventative screenings available
  • Listen and coach you on helath related issues
  • Conduct health risk assessments and assist in developing goals to improve your health and wellness
  • Guide women to age appropriate screenings and link them to providers
  • Provide follow-up and support after screening exams

Gail Cooper-Parks, RN, BSN, OCN, CBCN, Health Navigator, has more than 30 years of nursing experience. Gail has extensive experience in women's diagnostic imaging and outpatient imaging, and is certified as an Oncology Certifed Nurse (OCN) and Certified Breast Cancer Nurse (CBCN).

Throughout her career, Gail has developed and implemented new programs and initiatives that enhance patient care and quality of life. She provides education to patients and families with care and compassion, and is an extraordinary liaison. As Jupiter Medical Center's Health Navigator, Gail is ready and willing to help!
