JUPITER, Fla. (May 15, 2014) – Jupiter Medical Center has expanded its patient
services to include CareServices, a new home health partnership with CareSouth
Health System, Inc. The program, whose goal is to reduce readmission rates,
allows patients to recover and rehabilitate in the comfort of their own
homes after being discharged from the hospital.
CareSouth is one of the country’s leading providers of Medicare-certified
home health and other non-acute health services, providing quality, compassionate
in-home care services. The program provides at-home services including
nursing, home health aides, physical therapy, occupational therapy, speech
therapy, social workers, registered dieticians and home infusion services.
Certified nursing assistants and personal care aides are also available.
Readmission rates among CareServices patients are at 2.5 percent, compared
to 13.1 percent for the state of Florida and 12.1 percent nationally,
according to Strategic Healthcare Programs. This puts CareServices in
the 93rd percentile for excellence in the country.
The home health care provided by CareServices maintains the high-quality
care patients receive at the hospital. To qualify for services, it must
be deemed medically necessary, under a physician's plan of care, with
the patient and requiring one or more of the following: intermittent skilled
nursing, continued occupational therapy, physical therapy or speech language
pathology services. Eligible candidates are enrolled upon discharge from
Jupiter Medical Center, the Pavilion, or any hospital or Skilled Nursing Facility.
