Alzheimer's and Dementia Caregiver Support Program

Caring for a loved one with Alzheimer’s (Neurocognitive Disorders) can be rewarding, but also very challenging. At Jupiter Medical Center, we want to make sure that caregivers have the resources and support necessary to feel confident about caring for their loved one with Alzheimer’s Disease or any other Neurocognitive Disorders.

The Johnny and Terry Gray Alzheimer’s and Dementia Caregiver Support Program is committed to improving the experience of patients and families living with Alzheimer’s disease and related dementias by changing the way our healthcare system understands and treats these individuals. A resource for patients, families and clinicians, The Johnny and Terry Gray Alzheimer’s and Dementia Caregiver Support Program provides comprehensive education, clinical co-management services, and emotional and social support to all navigating the complex journey of dementia.
