Dementia Caregiver Navigation Program (Free)

The Dementia Caregiver Navigation Program focuses on comprehensive, coordinated dementia care and aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities.

  • Comprehensive Assessment: Patient and caregiver receive separate assessments to identify their needs and a home visit (if within 20 miles to hospital) to assess the patient’s safety
    • MoCA
    • Zarit Burden Index
    • Cohen-mansfield agitation inventory
  • Care Plan: Patients receive care plans that address their goals, preferences, and needs, which help them feel certain about next steps
  • Ongoing monitoring & Support: Care navigators provide monthly help to patients and their caregivers so they can revisit their goals and needs at any time and are not left alone in the process.
  • Referral & Support Coordination: Patients’ care navigator assists in connecting them and their caregiver to community based services and supports, such as home-delivered meals, transportation and day programs.
  • Caregiver Support: Caregivers take educational classes and can attend unlimited support groups both online and in person
  • Medication Management: Clinician reviews and reconciles medication as needed and care navigators provide tips for patients to maintain the correct medication schedule

Eligible Participants

  • Neurocognitive diagnosis (Alzheimer’s, Vascular dementia, Lewey-body dementia, Parkinson’s dementia, Frontotemporal dementia, etc.)
