you with the tools and resources you need to adopt a healthier lifestyle.
We have an experienced and caring team of professionals, state-of-the-art
facilities and wellness programs, classes and screenings to help you on
your journey to better health. We’ll be with you every step of the way!
Our Navigator can also assist if you have any health issues, guiding you
through the complex health care system, serving as a resource for you
and your family.
What Is a Navigator?
Our Health & Wellness Navigator is a registered nurse who works with
you to determine your health and wellness issues and goals. She will work
closely with you to help you develop strategies to achieve your goals.
Our goal is to help you “navigate” through a complex health
care system, ensuring you receive the right care at the right time.
Why Do I Need a Navigator?
Sometimes health care is confusing. You may have questions and would like
to talk with a health care professional you can trust, who will listen.
Sometimes you need help finding a medical specialist or need to understand
what screenings or tests will help you stay healthy. Our navigator is
just a phone call away.
What Services Does a Navigator Provide?
- Information on preventive screenings available
- Guide you to age appropriate screenings and link you to providers
- Provide follow up and support after screening exams
- Listen and coach you on health related issues
- Research questions and coordinate care related to medical conditions
- Connect you with area resources and act as an advocate
Jupiter Medical Center’s Navigator
Gail Cooper-Parks, RN, BSN, OCN, CBCN, Health and Wellness Navigator, has been a nurse for more than 30 years.
Gail has extensive experience in women’s diagnostic imaging, outpatient
imaging and is an Oncology Certified Nurse and Certified Breast Cancer
Nurse as well as a Health Coach. Throughout her career, Gail has developed
and implemented new programs and initiatives that enhance wellness, patient
care and quality of life. She provides education to patients and families
with care and compassion. She is an extraordinary listener and resource.
