JMC MyChart is a simple, secure and convenient way to request your electronic medical records.

For any issues with the patient portal or assistance with registration please email: patient.portal@jupitermed.com.

If you are requesting a change to your Health Information, please complete the Request for Correction/Amendment of Health Information form and email to: medical.record.corrections@jupitermed.com

If you would like a CD of your imaging films, please complete the Release of Information for Radiology. Bring the completed form to the Medical Records Department, please use the Outpatient Entrance accessed from Jupiter Lakes Blvd.

If you would like a copy of your medical records, you can request through MyChart, log on to your account and type in the browser address area: mychart.jupitermed.com/MyChartPRD/app/release-of-information to go directly to the medical records page. Messages are sent directly to the Medical Records Department. You can opt to complete the Release of Information form. Bring the completed form to the Medical Records Department. Effective September 24, 2025 The Medical Records Department has relocated to the hospital, please use the Outpatient Entrance accessed from Jupiter Lakes Blvd. The phone number is (561) 263-7417. The fax number is (561) 263-7416. Hours of operation are Monday - Friday, 8 am - 4:30 pm.

We encourage you to use our complimentary valet parking, you will need to go through security check-in, please have photo identification ready, when check-in is completed you will be directed to the medical records desk. Click here to view our campus map.

Historia clínica

Si usted fue paciente de Jupiter Medical Center y desea obtener una copia de su historia clínica, imprima y complete por escrito el formulario de Autorización para la Divulgación de Historia Clínica y siga las instrucciones detalladas en el Folleto sobre la Divulgación de Información de su Historia Clínica.

Si desea obtener una copia de sus estudios por imagen, complete el formulario de Divulgación de Información para Radiología.

Si desea realizar una modificación en su Información Médica, complete el formulario para la Solicitud de Correcciones/Modificaciones en su Información Médica.
