How to Continue Your Care at Home

We may all be patients in the hospital at some point in our lives. Because we are committed to the health, well-being and safety of our patients, we want you to be prepared when you are ready to be discharged from the hospital.

Home Health Services

A better way to care means treating the whole person, improved quality of life and superior results. At Jupiter Medical Center, in partnership with Encompass Health, our goal is to put patients first, reduce readmissions and allow patients to recover and rehabilitate in the comfort of their own homes after being discharged from the hospital.

Encompass Health will work collaboratively with your physician to to craft a plan of care that meets your specific needs. Their team of skilled nurses, physical therapists, occupational therapists, speech language pathogloists, medical social workers and home health aides use a coordinated interprofessional approach to deliver compassionate, specialized care in the comfort of your home.
